Hours of Operation:
Our medical center is open Monday through Friday from 8:30 am to 5:00 pm. Our physicians are on call 24 hours a day, 7 days a week.

 We currently offer a self-dispensing
medication service at our Medical Center at Plant City. With this service, we are able to offer our patients up to a
three-month supply of several commonly prescribed drugs right at the time of their appointment with their primary
care physician. Patients can leave the office with their prescriptions in hand, eliminating the need for a trip to the local pharmacy. |

Directions to Our Medical Center
Take exit 19 from I-4 (Thonotossassa Road) and go south to SR 92. Turn east to Alexander St. Turn south at light. The Continucare medical center is approximately two (2) miles on the left in Lake Walden Plaza.
